Supervision Information

This page is to share information with those that are supervising certified and provisionally licensed professionals as well as those that are being supervised. Follow the links below to get to forms and information on supervision.

Certificate holders and provisionally licensed professionals must be supervised by an approved Designated Qualified Clinical Supervisor (DQCS). You will receive confirmation, by email, from the Board office once your supervision agreement is approved. If you've not received confirmation within 7-14 days please contact the Board office. It is your responsibility to verify agreements have been approved. (Provisionally Licensed licensees will not be able to count hours towards licensure if an agreement is not approved by the Board.)

Current or Potential Supervisors:

If you are considering taking on the responsibility of supervising a provisionally licensed individual in the Mental Health Profession (or are already supervising someone), please click here for information on requirements and responsibilities involved.
