Effective May 15, 2023:  Due to the volume of verification requests, the Board office will not provide verification information over the phone or via email.  If you cannot access the form below you will need to submit the Verification Request form and fee for all verifications. 

The Mental Health Professions Licensing Board presents the License Directory information as a service to the public. The Board relies upon information provided by licensees to be true and correct, as required by statute. It is an act of unprofessional conduct for a licensee knowingly to provide erroneous information to the Board.
